Aging can be challenging. Recognizing that quality Care Management is an essential part of comprehensive healthcare, the Centers for Medicare and Medicaid Services (CMS) have created specialized programs to improve senior health and well-being.

These two programs are Chronic Care Management (CCM), and GUIDE (Guiding an Improved Dementia Experience).

CHRONIC CARE MANAGEMENT (CCM)
Chronic Care Management can serve as a critical component of healthcare for patients with multiple chronic conditions. Ongoing check-ins from the care team support closer adherence to treatment plans, leading to better health outcomes. This additional layer of oversight between health care visits can be instrumental in managing chronic conditions, improving quality of life for patients and their families.


GUIDE MODEL
Guiding an Improved Dementia Experience

GUIDE provides comprehensive Care Management for seniors with a dementia diagnosis who have Medicare as their primary insurance. This community-based model allows seniors and family caregivers to receive specialized support at home. GUIDE can transform the quality of life for individuals and families living with dementia. Services include:

  • COMPREHENSIVE ASSESSMENTS & CARE PLANS
  • COORDINATION & MANAGEMENT OF CARE
  • 24/7 TELEPHONIC ACCESS TO A TEAM MEMBER
  • RESPITE SERVICES
  • CAREGIVER SUPPORT
