- Safety in the Home – Assessment and plans to avoid injuries in the home.
- Assistance with ADLs (Activities of Daily Living) – Help with the many ways we take care of ourselves each day, such as: eating independently, dressing, bathing, using facilities, etc.
- Assistance with IADLs (Instrumental Activities of Daily Living) – Help with complex tasks: managing finances, shopping, medication dosages, housekeeping, cooking or food prep, transportation, laundry, shopping, and others.
- Coordination of Care Plan Services – Helping you with healthcare appointments, meals assistance, area resources, and more.
- Caregiver Support – GUIDE also focuses on the unpaid family members who care for their loved ones suffering from a dementia diagnosis.
